About the Project

LeaderShift is a multi-year, multi-sector initiative for current and emerging leaders in:
  • Team and Community-Based Primary Health Care
  • Home and Community Support Services
  • Community Mental Health and Addictions

The LeaderShift project partners are:
  • Association of Family Health Teams of Ontario
  • Addictions and Mental Health Ontario
  • Association of Ontario Health Centres
  • Canadian Mental Health Association
  • Ontario Community Support Association
 The LeaderShift project is the vision of Community Health Ontario (CHO). This alliance of four provincial associations believes that leaders in community and primary health care are well-positioned to make a critical contribution to an improved and more integrated health system.  Working collaboratively for over a year, the CHO Executives laid the groundwork and secured funding for this bold project which aims to:
  • foster linkages within and between community and primary health care organizations;
  • build leadership capability at all levels through cross-sector learning opportunities;
  • encourage leadership renewal as existing leaders prepare to retire;
  • inspire active leadership in system transformation.

Early in the planning, the CHO Executives were pleased to invite the Association of Family Health Teams of Ontario to join as a project partner.

The "LeaderShift" project is governed by Community Health Ontario (CHO), and managed by the Ontario Community Support Association. CHO gratefully acknowledges support for this project from the Ministry of Health and Long-Term Care.
