Ontario's vision for care at home relies on strong partnerships, local leadership, and a shared commitment to delivering integrated, patient-centred care. In this session, Cordelia Clarke Julien, Assistant Deputy Minister, Strategic Partnerships, Ministry of Health, will provide an update on provincial priorities, recent progress, and emerging opportunities in home and community care.

Drawing on insights and experiences from across Ontario, Cordelia will discuss how the province is advancing care closer to home to better meet growing demand and increasingly complex patient needs.
