Ontario’s primary care system is undergoing significant change, creating an important opportunity to build a more connected, team-based model of care. Home and community care providers must be more than participants in this transformation. They bring the trusted relationships, local knowledge, service expertise and practical understanding of client needs required to make it work.

This session will explore how home and community care organizations can position themselves as essential partners in primary care reform. Participants will consider opportunities to contribute at the governance, operational and service-delivery levels, from shaping regional priorities and building effective partnerships to redesigning pathways, coordinating services and supporting implementation on the ground.

Designed to move from ambition to action, the session will provide practical tools and frameworks that leaders can use to assess where their organizations can add the greatest value, strengthen their readiness for partnership and lead change within their communities.
