Session Description
Home and community care in Ontario is under mounting pressure: rising client acuity, constrained funding, workforce shortages and growing expectations from hospitals, clients, and families.
This panel brings together senior leaders to explore how community-based organizations are using data, digital infrastructure, and AI to move beyond activity-based reporting toward genuine outcomes measurement. Drawing on concrete experience with Hospital-to-Home programs, Ontario Health Teams, and high-intensity community supports, panelists will examine how community first digital innovation such as integrated data sharing, interoperability, and emerging AI tools is reshaping care coordination and strengthening integrated care— without losing sight of the human relationships at the centre of care.
Designed for senior leaders, executive directors, program managers, digital health leaders, and policy partners, this session provides practical insights into digital transformation grounded in operational experience rather than theory. Participants will leave with a practical framework for assessing digital readiness, strategies to strengthen collaboration across sectors, and new ideas for using data and technology to demonstrate the value and impact of community-based care. They will leave with at least one concrete action they can bring back to their own organization.
