From Pilot to Province: Scaling Integrated Home Care Through Ontario Health Team Collaboration

Session Description
How do you transform home care across an entire province? Across seven Ontario regions, healthcare partners have been testing new approaches to delivering home care that is faster, more coordinated, and easier for patients and families to navigate. While each region serves different populations and geographies, they all share a common goal: bringing providers together to deliver more seamless, person-centred care.
In this session, we will share lessons from Durham, Frontenac Lennox & Addington, Chatham-Kent, Nipissing, Mississauga, Guelph-Wellington, and East Toronto on how their integrated home care models were designed, implemented, and refined. Presenters, including the Health System Performance Network and the University of Toronto, will share early provincial evaluation findings, exploring what has improved, what challenges remain, and what lessons can help inform the next phase of home care transformation.
Designed for healthcare leaders, Ontario Health Teams, policymakers, and community care providers, this session offers a unique opportunity to compare implementation across urban, rural, and northern communities. Participants will leave with practical frameworks, evaluation insights, and implementation tools that can help strengthen integration efforts within their own organizations and regions.
