Helping Seniors Age in Place: The Power of Community-based, Group Exercise and Falls Prevention Programs

Session Description

Falls remain one of the leading causes of injury, hospitalization, and loss of independence among older adults. As more people choose to age at home, community-based exercise and falls prevention programs are becoming an increasingly important part of helping seniors remain active, confident, and connected while reducing avoidable pressure on the healthcare system. 

 

This interactive session explores how Circle of Care has designed and implemented a community-based Exercise and Falls Prevention Program that brings exercise science-informed, ADL-focused programming directly into neighbourhoods through community centres, multi-unit residences, and other local settings. Presenters will share practical lessons on program design, participant engagement, site selection, staffing, and evaluation, along with findings from a recent three-month evaluation demonstrating the program's impact on physical function and participant experience. Participants will also be invited to share their own experiences, discuss common implementation challenges, and explore opportunities to strengthen and expand falls prevention initiatives across Ontario. 

 

Designed for program managers, organizational leaders, and system partners interested in healthy aging and preventative care, this session provides practical strategies, evaluation tools, and implementation insights that can be adapted across community settings. Participants will leave with new ideas to strengthen local programming, build partnerships, and advocate for greater investment in preventative approaches that help older adults live safely and independently at home.
