Upstream Impact by Design: Strengthening Integration Between Primary and Home & Community Care

As Ontario moves forward with primary care reform, there is growing recognition that home and community care must be a key partner in building a more connected and prevention-focused health system. This plenary session, in partnership with Santis Health, explores how Ontario’s home and community care sector can help advance this vision. It will outline government priorities for the Ministry of Health and the evolving role of Ontario Health Teams.

 

With insights from OCSA members who have partnered with primary care providers and Santis Health experts, the session will explore real-world models, lessons learned, and practical strategies to support integration. It will showcase promising innovations that connect clinical care with home and community-based supports to improve patient outcomes. The session will conclude with an interactive Q&A.
